MADISON — Indiana regulators have fined a construction company $10,000 for violations found following the death of an Ohio River bridge worker.

Fifty-year-old Roger Cox of Austin, Ind., died in May after becoming trapped between an aerial bucket he had been standing in and a steel beam on the bridge linking Madison, Ind., and Milton, Ky. A coroner ruled Cox died due to a lack of oxygen reaching his brain.

The Madison Courier reports that the Indiana Occupational Safety and Health Administration cited Walsh Construction for two violations, including a failure to offer proper training to employees using the aerial lift. The agency also found that Cox had not been properly trained to use the aerial lift he was working from.

Walsh Construction was fined $5,000 for each violation.
