There was the loss of a wheel assembly, which sent the sport utility vehicle off the side of the road and into a ditch.
Thatís when it began rolling. And thatís when a woman was ejected and killed while four others inside Ė including a 1-year-old Ė were hurt.
Indiana State Police are piecing together what happened on U.S. 27 just south of County Road 850 N. in Adams County on Sunday, but troopers have completed a preliminary investigation.
A 1998 Ford Expedition driven by a man identified as 47-year-old Tyler Cowans, of Decatur, was going south on U.S. 27 at about 4 p.m. when the SUVís rear wheel assembly came off.
Cowans lost control and the SUV went off the west side of the road, rolling into a ditch. Cowans was not wearing a seatbelt.
Neither was the woman sitting next to him in the passenger seat, police said. The woman was ejected from the SUV. Emergency responders pronounced her dead at the scene. Her identity has not been released. An autopsy has been scheduled.
Cowans suffered head and internal injuries and was transported to a hospital in Fort Wayne. An update on his condition was not available.
Three others in the SUV were also injured and taken to a Fort Wayne hospital suffering from internal injuries. Updates on their conditions have not been released.
They were identified as: Maria Cowans, 19, of Decatur; Rick Stults, 18, of Geneva; and Jaiden Cowans, 1, of Decatur.
The crash is under investigation.
